Once completed you may fax or scan it directly to our accounting department: 703.516.9308 or accountsreceivable@usaretirement.org.
When will I receive my certificate?
Once you become credentialed, you will be sent a digital certificate within 1 week. You will be sent a hard copy certificate within 3 months to the address on file.
